Important: Field names in Tally MUST match Notion property names exactly.

   • Field Name: Full Name (spaces matter!)
   • Required: Yes

   • Field Type: Short text
   • Field Name: Company
   • Required: No
4. Question: "Phone number (optional)"
   • Field Type: Phone

   • Required: No
5. Question: "How did you hear about us?"
   • Field Type: Multiple choice
   • Field Name: Lead Source
   • Options: Website, Referral, Social Media, Event, Other
   • Required: Yes
6. Question: "Tell us about your project/needs"
   • Field Type: Long text

• After submission: Show thank you message
• Collect email addresses: Enabled
• Allow multiple submissions: Disabled
